If you were selected for additional verification for receiving Financial Aid, you will need to complete one or more additional forms.

You will be assigned the forms on your To Do list in ctcLink (see desktop directions here and mobile directions here).

2024-25 Verification Forms

Please contact the Financial Aid Office at [email protected] before completing these forms.  Some of these forms require being completed in person in the Financial Aid office or by a notary.

What you need to complete the verification forms

V1 Independent – V1 forms verify number in household, number in college, and tax information. If you used the IRS Data Retrieval Tool, you only need to complete the form according to the instructions. If you did not/could not use the DRT, you will need to provide tax information for yourself (and spouse, if applicable) in addition to the form. Tax documents can be provided in the form of an IRS Tax Transcript or a signed copy of the 1040 filed. If you (or your spouse, if applicable) were not required to file taxes, you will need to provide an IRS Verification of Non-Filing letter.

V1 Dependent – V1 forms verify number in household, number in college, and tax information. If you and/or your parents used the IRS Data Retrieval Tool, you only need to complete the form according to the instructions. If you did not/could not use the DRT, you will need to provide tax information for yourself and/or your parents in addition to the form. Tax documents can be provided in the form of an IRS Tax Transcript or a signed copy of the 1040 filed. If you and/or parents were not required to file taxes, you will need to provide an IRS Verification of Non-Filing letter. Dependent students are allowed to write their own statement rather than requesting an letter form the IRS.

V4 – V4 forms required a signed Statement of Educational Purpose. They must have a “wet” signature, so we are unable to accept these forms digitally. You can either sign the Statement in front of one of our Financial Aid staff members, or have the form notarized and mail it to us. We are required to include a copy of your valid photo ID – please bring your ID with you if you are signing in person.

V5 Dependent/Independent – V5 includes all the requirements from both V1 and V4 forms. Please see the above instructions.

Financial Aid Suspension Appeal Form

Note: Completed appeals are due by the end of the first Friday of the quarter for which you are seeking reinstatement.

Enrollment Change Form

  • Enrollment Change Form for Spring 2025
    The deadline to submit an enrollment change form is 7 days prior to the start of the quarter. The winter quarter deadline is March 30, 2025, at 11:59 p.m.

This form is to report changes or updates regarding enrollment so financial aid awards can be adjusted accordingly. This is required for students registering less than full-time. It is also important for those returning after a break in attendance, or updating previously adjusted enrollment. Students who have previously had aid cancelled due to lack of or insufficient enrollment should also complete this form. Incorrect information can cause a delay in aid or an overpayment that will need to be returned.

Federal Tax Information Release Form

This form allows us to release Federal Tax Information imported from the IRS to be shared with programs/individuals outside of the Financial Aid Office – please do not complete this form unless you have been instructed to do so.
