Uniform Medical Plan Classic

Uniform Medical Plan is a preferred provider network. That means that you can choose your own provider and you don’t need to get a referral from UMP to see a specialist. You will receive the highest level of benefit and pay the lowest amount out of your pocket if you see network providers.

Uniform Medical Plan Select

Uniform Medical Plan Select (UMP Select) is a self-insured Preferred Provider Organization (PPO) health plan. UMP is offered through the Washington State Health Care Authority’s Public Employees Benefits Board (PEBB) Program. UMP is administered by Regence BlueShield and Washington State Rx Services (WSRxS). All prescription drugs, services, or other benefit changes may require approval by the PEB Board. Approval takes place when benefits are procured for the next calendar year.

Uniform Medical Plan CDHP

This Uniform high deductible plan has a low monthly premium and a high deductible.

You cannot have both a Limited Purpose FSA and a Medical FSA, nor can you have a Medical FSA and be enrolled in a consumer-directed health plan (CDHP) with a health savings account (HSA).

  • If you enroll in both a Medical FSA and a consumer-directed health plan (CDHP)with a health savings account (HSA) in the same year, you will be automatically disenrolled from the Medical FSA.

If you have a CDHP with an HSA you can enroll in a Limited Purpose FSA for eligible dental and vision expenses.

Please note that you need to submit a Limited Use VEBA if you are an exempt employee and enrolled in this plan.

Kaiser WA Classic

The Classic plan for PEBB employees offers the lowest co-payments.

Kaiser WA Value

If you’re concerned about monthly costs, the Value plan for PEBB employees has the lowest premium you can get. Get all of the same valuable services as the Classic plan, while keeping more in your paychecks. Group Health Value has a deductible and the co-pays are a little higher.

Kaiser WA CDHP

This Group Health high deductible plan has low monthly premiums and a high deductible.

You cannot have both a Limited Purpose FSA and a Medical FSA, nor can you have a Medical FSA and be enrolled in a consumer-directed health plan (CDHP) with a health savings account (HSA).

Uniform Dental Plan (Group #3000)

The Uniform Dental Plan is a preferred provider plan administered by Washington Dental Plan that allows you the freedom to choose any dentist. However, it provides a higher level of reimbursement if your dentist is a WDS participating provider. The UDP offers services in every county of Washington State. Outside of Washington, services are reimbursed at a higher level than for services provided by non-PPO dentists in Washington. You can verify your dentist’s participation by calling Uniform Dental Plan at 1-800-537-3406.

Note: UDP does not issue ID cards.

Willamette Dental Plan

Willamette Dental Plan (a new dental plan that offers the same provider network as Regence BlueShield Columbia Dental Plan), requires that you receive care from Willamette Dental dentists or specialists. Their dental offices are located in Bellevue, Bellingham, Everett, Federal Way, Kennewick, Kent, Lakewood, Longview, Lynnwood, Olympia, Pullman, Puyallup, Renton, Richland, Seattle, Silverdale, Spokane, Tacoma, Tumwater, Vancouver, Wenatchee, Yakima, and Portland metro area (Oregon). You can contact the provider at 1-855-433-6825.

Note – Willamette Dental does not issue ID Cards

DeltaCare (Group #3100)

DeltaCare (Group #3100), administered by WDS, you select a primary care dentist from the DeltaCare network, and you must receive care from your selected dentist. Because WDS administers several dental plans, each with its own provider network, be sure to verify that your dentist is in the DeltaCare network. This is important, as you could be responsible for costs if you receive care from a provider who is not in the DeltaCare network. Providers are located in: Aberdeen, Arlington, Auburn, Bellevue, Bellingham, Bonney Lake, Bothell, Bremerton, Burien, Covington, Des Moines, Edmonds, Everett, Federal Way, Gig Harbor, Issaquah, Kennewick, Kent, Kirkland, Lacey, Lake Stevens, Lakewood, Lynnwood, Maple Valley, Marysville, Mercer Island, Mill Creek, Moses Lake, Mt Vernon, Mukilteo, Olympia, Poulsbo, Puyallup, Renton, Seattle, Shoreline, Silverdale, Snohomish, Spokane, Spokane Valley, Stanwood, Tacoma, Tukwila, Tumwater, University Place, Vancouver, Wenatchee, Yakima, Clackamas, Hillsboro, Portland. Note – Delta Care does not issue ID Cards. They can be printed by registering on Delta Dental’s website.

Long-term Disability

Basic Plan

This plan is included in the full PEBB package at no additional cost for the employees. The Basic Plan provides a benefit of 60 percent of the first $400 of pre-disability earnings, reduced by any deductible income. The maximum benefit payable is $240 per month. The minimum benefit is $50 per month. Benefits begin after 90 days of total disability or after the period of your accumulated sick leave, whichever period is longer, and continue during your disability up to the Maximum Benefit Period. The Maximum Benefit Period is determined by the type of disability and the age of the enrollee when he or she becomes disabled.

Optional Plan

This plan allows most employees eligible for the Basic Plan to apply for additional benefits.  If the coverage is applied for within 31 days of your eligibility date, you don’t need to provide evidence of insurability.  When combined with the Basic Plan benefits, the Optional Plan will pay 60 percent of the first $10,000 of your pre-disability earnings, reduced by any deductible income.  The minimum combined benefit is $100 per month.  Optional Plan benefits begin after the end of the Benefit Waiting Period and continue during disability up to the Maximum Benefit Period.

Life Insurance

The PEBB Program’s full employee benefits package includes basic life and basic AD&D insurance to eligible members through Metropolitan Life Insurance Company (MetLife). As an employee, your basic life insurance covers you and pays your designated beneficiaries in the event of your death. Basic AD&D insurance provides extra benefits for certain injuries or death resulting from a covered accident.

You will be automatically enrolled in these benefits, even if you waive medical coverage. You do not need to provide evidence of insurability (proof of good health).

These benefits provide:

  • $35,000 for basic life insurance.
  • $5,000 for basic AD&D insurance.

You can also enroll in supplemental life and supplemental AD&D insurance for yourself and your eligible dependents. To enroll your eligible dependents in supplemental life and AD&D insurance, you must elect employee supplemental life insurance for yourself.

Aflac

Aflac offers supplemental, dental, vision, cancer, disability, and other insurance policies as well as employee benefits for individuals. If you get hurt and miss work, Aflac is there.  Aflac has a policy for just about any need and just about any budget. Aflac is available through payroll deduction.

Combined Fund

For over 23 years the Washington State Combined Fund Drive has set the standard of giving amongst similar giving programs.  Each year, active and retired public employees participate in the annual Combined Fund money for local, national, and international communities.  With the help of the incredible volunteers, and the generous gifts from every participant, the CFD now raises over $5.5 million per year.

The Combined Fund Drive offers the security and convenience you expect when making a donation to your favorite charity.  You can choose a monthly payroll deduction, write a check, go online, make a one time pledge, or fill out a paper form.  Combined Fund Drive is here to make your charitable donation experience as easy and enjoyable as possible.

Dependent Care Assistance Program

As a Washington state employee, you may be eligible to take advantage of the Dependent Care Program (DCAP)  A DCAP account allow syou to pay for child, elder, or other dependent care expenses by setting aside a portion of you payroll dollars on a pre-tax basis and reducing your taxable income.

When you enroll in DCAP, you decide how much to contribute for the entire plan year (January 1 – December 31).  The money is deducted from your paycheck pre-tax (before federal income taxes and FICA taxes are deducted) in equal amounts over the course of the plan year.  After you incur qualified expenses, you submit a claim to Navia requesting reimbursement from your DCAP account.  If you incur child, elder, or other dependent care expenses, you should consider this excellent employee benefit.

IMPORTANT NOTE – USE IT OR LOSE IT

Reimbursable expenses in the DCAP program must be incurred during the eligible plan year AND submitted to Navia no later than March 31 of the following calendar year.  Any unclaimed dollars remining in your account will be forfeited to the State of Washington, per IRS regulations.

Eligibility Requirements

To be eligible for the DCAP, you must be eligible for PEBB health benefits.  DCAP can only be elected during open enrollment or within the first 31 days of new employment.

Guaranteed Education Tuition Program

The Guaranteed Education Tuition Program is Washington’s 529 prepaid college tuition program.  With GET, you prepay for your child’s college tuition today.  Your money is guaranteed by the State of Washington to keep pace with rising college tuition, no matter how much it increases in the future.

You don’t have to worry about the ups and downs of the stock market or whether you’ve saved enough.  And you have peace of mind knowing that your money will be there when your child is ready for college.

Choice of colleges nationwide

You can use your GET account at nearly any public or private college in the country as well as some schools around the world.  The value of your account is the same, whether your child attends a community college close to home or a private college in another state.

If tuition at your child’s college is lower than the resident undergraduate tuition and state-mandated fees at the UW or WSU, you can use your extra units for room and board, books, or other qualified higher education expenses.  If your child’s tuition is higher, you’ll pay the difference.

Tax-free savings and withdrawals

Because the GET Program is a state 529 plan, the after-tax money you put into your account will grow tax-free.  And, when your child is ready for college, the money you withdraw will be tax-free as long as you use it for qualified higher education expenses.

Flexibility and control

You can use your account for tuition, room and board, books or other qualified higher education expenses.  If your child doesn’t go to college or receives a scholarship, you can transfer your account to another family member or request a refund.  And, unlike some other savings options, you maintain total control over your account.

Easy ways to save

You can set up a monthly payment plan, send a check whenever you have extra cash, or choose a combination of the two.
You can open an account for anyone–your child, grandchild, friend or even yourself.  The only requirement is that either the account owner or student be a Washington resident at the time of enrollment.

Flexible Spending Account

A Healthcare Flexible Spending Account (FSA) is a reimbursement account that allows you to set aside money for healthcare costs from your paycheck before taxes are taken out. The advantage is that you’re spending pretax dollars, so you end up paying fewer taxes on your salary. With an FSA, you can be reimbursed for dollars spend on eligible healthcare expenses that are not paid for by your health insurance plan.

IMPORTANT NOTE – USE IT OR LOSE IT

If you are enrolled in an FSA on the last day of the plan year (December 31), you have a grace period from January 1 – March 15 to incur expenses and use funds from the previous year. Navia must receive your claims no later than March 31 to pay claims from the previous plan year’s FSA account.  Any unused funds left in your account on March 31 will be forfeited to the Health Care Authority, as required by IRS regulations.

Eligibility Requirements
