A Student Travel Reporting Form is required for each trip and for each hotel or lodging facility that is utilized on each trip. For example, if you spent three nights on a trip and stay in three different hotels or lodging facilities, then three separate Student Travel Reporting Forms are required.

The Campus Safety and Security Department maintains a list of all submitted Student Travel Reporting Forms to aid in compliance with the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act.
